Consent Form - Allergy Test & Beauty Consultation Records

At Syer Hair & Beauty Salon in Sutton Coldfield, we take your privacy seriously.

For the safety of our clients, we maintain records of any health or medical conditions which may indicate that we should not go ahead with a particular service or treatment. For example, we will keep records of any allergies you may have, whether you are pregnant, and whether you have a skin condition.

These records ensure we give you safe and appropriate treatments and use the correct products for your needs. For example, if you are allergic to products containing nuts or fish oils, we would make a note of this to ensure we use an alternative product.

These health records are not used for any other purpose. Client records are held securely within our salon software system and can only be seen by members of the salon staff.
